Kohima, July 5 (MExN): The Clairvoyance Technologies Ltd will be conducting PMJAY beneficiary verification and issue of e-card under Kohima municipal area from July 9.
This will be carried out under the banner of State Health Agency (AB-PMJAY), Directorate of Health & Family Welfare, Nagaland.
In this respect, all the ward authorities have been requested to arrange enrolment station and also make necessary announcement to the public to come to the enrolment station on the appointed day along with Aadhaar Card or Voter ID or Pan Card or any government issued ID Card, Ration Card or Letter from Village/ Ward Council clearly listing out all the members in the household with photos pasted and signed across by Chairman Panchayat/Village Council and RSBY 2016-17 card for RSBY card holders.
A release received here stated that such documents are required to check their eligibility and enrolment of eligible beneficiaries.
Schedule for PMJAY: July 9- Naga Bazaar, Kitsubozou and D’ Block, July 11- Daklane, Midland and PWD, July 13- Upper Chandmari, Potterlane and AG Colony, July 16- Para Medical and PR Hill and July 18- High School, Bayavu and Kezieke.
