Kohima, August 5 (MExN): Clairvoyance Technologies Pvt. Ltd will be conducting another phase of PMJAY beneficiary verification and issue of e-card for Kohima district on behalf of the State Health Agency (AB-PMJAY) from August 10.
All the village authorities have been requested to arrange enrolment station and also make necessary announcement to the public to come to the enrolment station on the appointed day along with the following documents to check their eligibility and enrolment of eligible beneficiaries: Personal ID:- Aadhar Card or Voter ID or any Government issued ID, Family ID:- Ration Card or Letter from Village/ Ward council clearly listing out all the family members in the household with photos pasted and signed across by chairman Panchayat/Village Council ( Format to be issued by the enrolment team) and RSBY 2016-17 card for RSBY card holders.
Schedule for PMJAY (Kohima district): August 10- Khuzama and Kigwena town, August 12- Viswema, August 14- Rusoma & Chedema Model, August 17- Phesama & Pfuchama, August 19- Kidima and August 21– Kezoma town, Kezoma village and Sakhabama.
