Dimapur, February 5 (DIPR): Deputy Commissioner and District Election Officer, Dimapur, Sushil Kumar Patel, IAS, has in a circular informed all Government servants requisitioned for election duty to the 13th Nagaland Legislative Assembly under Dimapur district to avail Form 12 (application for Postal Ballot) at Revenue Branch 1st Floor, DC office, Dimapur.
The circular informed all personnel drafted for election duty and desirous to vote through postal ballot to collect Form 12 from the above mentioned address and submit the completed form on or before 10/02/18 along with the following documents: EPIC or service ID card and Election duty appointment order.
While observing that many applicants have submitted Form 12 without the requisite documents or without indicating Sl No, Part No and A/C details Sushil Kumar cautioned that such incomplete applications are liable to be rejected for issue of Postal ballot, and requested all concern to ensure that the submitted forms are completed with the requisite documents.
