Kohima, March 25 (MExN): Two-day disability assessment camp for registration in the UDID portal and to digitise the disability certificate will be held at Dr Imkongliba Memorial Hospital, Mokokchung on March 29 and 30.
The camp is organised by Department of Social Welfare under the Ministry of Social Justice and Empowerment in collaboration with the Health & Family Welfare Department under UDID Project.
Therefore, the department has notified that all Persons with Disability who have not yet obtained disability certificate to bring one passport size photo, Id and address proof (Aadhar Card, Ration Card, residential Certificate etc) to register under UDID Portal for issue of Unique Disability Id Card.
PwDs who have already obtained disability certificate from the District Hospital need not come for the camp but can submit their documents online at www.swavlamban.gov.in or at their respective District Welfare Offices, it stated.
