Mon/Phek, February 3 (DIPR): K Thavaseelan (IAS), Deputy Commissioner & District Election Officer of Mon has notified to all concerned that Form 12 will be made available at the Postal Ballot Facilitation Centre, 1st Floor, DC office, Mon starting February 3. ID cards for verification are to be produced to avail the forms. Last date of submission of forms will be February 8 for the personnel who are enrolled in other districts. For personnel who are enrolled under Mon district, the last date of submission is February 13. Office of the Deputy Commissioner and District Election Officer, Phek has also informed all concerned employees drafted for election duty to submit duly filled in Form 12 along with photocopy of appointment order and EPIC card to the Postal Ballot Facilitation Centre on or before February 8 to enable the office to make necessary arrangement for issue of postal ballot papers to all the applicants.
