
DIMAPUR, MAY 3 (MExN): The Commissioner of Disabilities, State of Nagaland in collaboration with the Nagaland Parents Association for the Disabled (NAPAD) today drew attention of persons with disabilities and parents/ legal guardians / relatives / NGOs of persons with disabilities of Kohima District to a health insurance cover scheme of the government. A notification informed that the Swavlamban Health Insurance Scheme under the Department of Empowerment of Persons with Disabilities, Ministry of Social Justice and Empowerment, Government of India provides affordable health insurance cover to persons with disabilities through the New India Assurance Co.Ltd. In this regard, the Commissioner of Disabilities, State of Nagaland and the NAPAD informed all the eligible persons with disabilities like Blindness, Low Vision, Leprosy –Cured, Hearing Impairment, Loco- Motor Disabilities, Mental Retardation and Mental Illness (Persons with multiple Disabilities, Cerebral Palsy and Autism are not covered under this Scheme) to avail the scheme. The forms are available at the NAPAD Office, below Sakhrie Park, AG Colony, Kohima. The documents required are: duly filled and signed Proposal Form; 2 (Two) Passport size Photographs for each family member and copy of any Government ID Proof i.e. Aadhar Card, Voters ID, Driving License etc; Proposer required to pay only 10% of the total premium of Rs.3100/- + Service Tax under the Scheme i.e. Rs. 360/- ( Rs. 310/- + Service Tax); Cancelled Cheque copy or Bank Account No. with IFSC Code; and Disability Certificate. The last date for submission of Proposal Form along with all the required documents is May 20, 2017.
